ASSENT TO PARTICIPATE IN RESEARCH
Vanished: A Science Mystery Game
1. My name is Scot Osterweil, Research and Creative Director at the MIT Education Arcade.
2. We are conducting a study on Vanished to learn more about how well our games do with players of your age. We will be collecting some data from all Vanished players. If you play Vanished, we will be collecting some data from you as well.
3. If you play Vanished, we will ask you some survey questions on the website about how you feel about science and how well the game is going for you. These questions will be used to figure out if we need to improve our game. The questions are for testing the game, not for testing you. While you use the website, we will also collect some data about when and how you use the site. This data will be collected automatically – you won’t notice it being collected.
4. While playing Vanished, you might feel like you aren’t doing well in the game, and you may stop playing at any time.
5. Please talk this over with your parents before you decide whether or not to participate. We will also ask your parents to give their permission for you to take part in this study. But even if your parents say “yes” you can still decide not to do this.
6. We will collect data from all Vanished players. So if you don’t want us to collect data from you, you should not play Vanished. Remember, playing this game is up to you and no one will be upset if you don’t want to participate or even if you change your mind later and want to stop.
7. You can ask me questions that you have about the study now by emailing questions@vanished.mit.edu. If you have a question later that you didn’t think of now, you can email me later. You can also call the Chairman of the Committee on the Use of Humans as Experimental Subjects at M.I.T. at 1-617-253 6787 if you feel you have been treated unfairly.
8. Clicking “Accept Terms & Conditions of Use” at the bottom of this page means that you agree to play Vanished and allow us to collect data from you. We will email you and your parents a copy of this form if you agree to play.
